Christmas Angels
As the holiday season draws near, AllThrive 365 is blessed to have incredibly generous members in our community ready to spread some festive cheer. If you have clients who would love a holiday gift, be sure to complete and submit this form on their behalf by the end of the day on Thursday, October 8, 2026. All gift requests must be made by October 8, 2026.
Form Submission Instructions:
- This program is eligible for AllThrive 365 clients only.
- Please submit one form per client.
- You may request up to two items ONLY totaling no more than $30 for each client.
- Please be as specific as possible about what gift your client wants, including sizing if applicable, and whether the individual is male or female.
- All gifts will be wrapped and dropped off at the 1201 office by 12/7.
- All gifts will be wrapped and available for pick up by the week of 12/14/26 in the ABC room at 1201 E Thomas Rd. Phoenix, AZ. 85014. Abigail de Leon will coordinate with Tempe and Glendale ADHS locations for drop off. Behavioral Health Home staff will pick up at AllThrive 365’s corporate office.
